MY1Life Client Agreement
These terms apply to every MY1Life coaching program. "MY1Life", "we" and "us" mean MY1LIFE PTE LTD (UEN 202446563R). "Coach" means May Young. "You" means the client. Please read them before you enrol; questions are welcome at may@my1life.co.
1. Coaching Agreement
1.1 What coaching is. MY1Life provides health education and coaching on blood sugar, nutrition, movement, sleep, stress and related habits. Coaching helps you understand your own data and build skills to make your own decisions.
1.2 What coaching is not. The Coach is not a doctor, dietitian or other licensed healthcare professional. MY1Life does not diagnose, treat, cure or prevent any disease, and does not prescribe or change medicine. Nothing in coaching replaces advice from your doctor.
1.3 Your program. Your program is the one you choose and pay for: CGM Decode (15 days), Metabolic Reset (8 or 12 weeks) or Healthspan 6 (6 months). Its contents are as described on the MY1Life programs page at the time you enrol.
1.4 Program period. Your program starts on the date of your first call. Sessions must be used within the program length plus 4 weeks (for example, an 8-week program within 12 weeks of the first call). Unused sessions after that lapse and are not refunded.
1.5 Calls and rescheduling.
- Calls take place online by video, at times agreed between you and the Coach.
- To reschedule, give at least 24 hours' notice. A call missed or cancelled with less notice counts as used.
- If you are more than 15 minutes late, the call may be shortened or count as used.
- If the Coach needs to reschedule, you will be offered a new time at no cost.
1.6 Between calls. You may message the Coach on weekdays through the agreed channel. Replies normally come within 1 working day. Messaging is not monitored around the clock and must never be used for urgent or emergency health matters.
1.7 Your responsibilities. You agree to:
- give accurate and complete information about your health, medicines and history, and tell the Coach promptly about any changes;
- keep your doctor informed that you are working with a coach and wearing a CGM;
- make any change to medicine only with your doctor;
- take part actively; results depend on what you put into practice.
1.8 No guaranteed results. Everyone's body is different. MY1Life does not promise any particular result, such as weight loss, lower HbA1c or coming off medicine.
1.9 Materials. Guides, templates, recordings and your Personal Metabolic Playbook are for your personal use. Please don't share, sell or publish them. MY1Life keeps the rights to its materials and methods.
1.10 Respect and conduct. Both sides agree to treat each other with courtesy. MY1Life may end the program if a client is abusive or harassing, or repeatedly breaks the community group's house rules.
1.11 When coaching stops for health reasons. If the Coach believes coaching is not safe or suitable for you (for example, because of a new diagnosis or a medicine that needs closer medical supervision), the Coach may pause or end the program and ask you to see your doctor. Refunds in this case follow section 2.5.
1.12 General. This agreement, the program description and the policies below are the whole agreement between you and MY1Life. If any part is found unenforceable, the rest still applies. This agreement is governed by the laws of Singapore.
2. Fees, payment and refunds
2.1 Fees. The fee for your program is shown on the programs page when you enrol. For clients in Singapore, Malaysia, Vietnam and Indonesia, fees are in Singapore dollars and include every CGM sensor for your program: 1 for the CGM Decode, 4 for the 8-week Metabolic Reset, 6 for the 12-week Metabolic Reset and 12 for Healthspan 6. Outside Singapore, shipping is added at cost and shown on your invoice. Clients in other countries pay in US dollars and buy the same number of sensors themselves, from a supplier in their own country. Add-ons are charged separately. Blood tests are not included: you order them through your own doctor, or the Coach can refer you to a partner clinic, at your own cost.
2.2 Payment in full, before you start. Pay the full fee before your first call. Your place is confirmed once payment is received, and you'll get a receipt by email. There are no instalment plans.
- Singapore clients: PayNow to MY1LIFE PTE LTD (UEN 202446563R), with your payment reference.
- Clients outside Singapore: bank transfer to MY1LIFE PTE LTD, using the details on your invoice. We recommend Wise to keep fees low. You pay any transfer or bank fees, and the amount received must equal the invoiced fee.
2.3 Cancelling before you start.
- More than 7 days before your first call: full refund, less S$38 for each CGM sensor already sent to you.
- 7 days or fewer before your first call: 50% refund, less the cost of sensors already sent; or move your start date once, within 2 months, at no cost.
2.4 After your first call. Fees are not refundable once the program has started, because time, materials and sensors have been committed to you. You may pause the program once, for up to 4 weeks, for illness, travel or family reasons; the program period is extended by the length of the pause.
2.5 If MY1Life ends the program. If the Coach ends the program for health or safety reasons (section 1.11), or cannot continue to coach you, you'll get a refund for the unused part of the program, worked out by the number of sessions left. If the program ends because of a breach of the conduct terms (section 1.10), no refund is due.
2.6 CGM sensors. Sensors are non-refundable once applied. A sensor that fails before the end of its wear period will be raised with the manufacturer for replacement under its own policy; MY1Life will guide you with the claim.
2.7 Refund payment. Approved refunds are paid within 14 days, by PayNow for Singapore clients or by bank transfer for clients outside Singapore.
2.8 Decode credit. If you complete a CGM Decode and join a Metabolic Reset or Healthspan 6 within 30 days of your Decode results session, the Decode fee is taken off your new program's fee.
3. Health disclaimer, risks and release
3.1 Talk to your doctor first. Before starting, and before changing how you eat, fast or exercise, speak to your doctor. This is especially important if you:
- take insulin or any medicine that lowers blood sugar, or medicine for blood pressure;
- have type 1 diabetes, kidney disease, heart disease, or a history of low blood sugar;
- are pregnant, trying to conceive or breastfeeding;
- are having, or have recently finished, cancer treatment;
- have, or have had, an eating disorder.
3.2 Known risks. Changes to eating, fasting and exercise can cause side effects. These include low blood sugar (shakiness, sweating, confusion, fainting), dizziness, tiredness, muscle soreness or injury, and changes in how your medicine works. Stop and seek medical help if you feel unwell.
3.3 Emergencies. Coaching is not an emergency service. In an emergency in Singapore, call 995 or go to the nearest Emergency Department. Overseas, call your local emergency number.
3.4 Your health information. You confirm that the health information you give MY1Life is true and complete to the best of your knowledge, and that you'll tell the Coach promptly about any change, new diagnosis or new medicine.
3.5 Your choices. You decide whether to follow any suggestion. You take part voluntarily and accept the risks described above that are normal for changes to diet, exercise and lifestyle.
3.6 Release and limit of liability. To the extent the law allows, you release MY1Life and the Coach from claims arising from your own decisions, from information you did not disclose, and from following suggestions without consulting your doctor where this agreement asks you to. MY1Life's total liability to you for any claim related to this agreement is limited to the fees you paid for your program.
3.7 What this does not limit. Nothing in this agreement excludes or limits liability for death or personal injury caused by negligence, for fraud, or for anything else that cannot be excluded or limited under Singapore law.
4. CGM use acknowledgement
By wearing a continuous glucose monitor (CGM) during your program, you understand that:
- It's for learning, not diagnosis. In this program, the CGM is used to learn how your body responds to food, movement, sleep and stress. Its readings are not used to diagnose any condition.
- Never use it to dose medicine. Don't change insulin or any other medicine based on CGM readings unless your doctor has told you to.
- Readings can differ from a finger-prick or lab test, especially in the first day, after strenuous exercise, or when the sensor is pressed while you sleep. If your readings don't match how you feel, trust how you feel and check with a finger-prick meter or your doctor.
- Follow the maker's instructions for applying, wearing and removing the sensor, including advice on X-rays, MRI and CT scans, and on certain medicines or supplements (such as high-dose vitamin C) that can affect readings.
- Skin reactions can happen. Remove the sensor and see a doctor if you notice pain, swelling, warmth or a rash that doesn't settle.
- Each sensor lasts 15 days and is single use. In the Metabolic Reset and Healthspan 6 you wear one continuously, putting on a new sensor as each one ends. Dispose of used sensors as the maker advises.
- Log your meals in the CGM app during your program, so your reports can show how each meal affects you.
- Sharing your data: you'll share your CGM reports with the Coach, either through the app's sharing feature or as an exported report. You can stop sharing at any time.
5. Privacy notice (PDPA)
MY1Life follows Singapore's Personal Data Protection Act 2012 (PDPA). This notice explains what we collect, why, and your choices.
5.1 Who looks after your data. MY1LIFE PTE LTD (UEN 202446563R), Singapore. Data Protection Officer: May Young, may@my1life.co.
5.2 What we collect.
- Contact details: name, email, phone, country, time zone.
- Health information: goals, health and family history, medicines and supplements, symptoms, blood test results and reports, body measurements, CGM data, eating, sleep and exercise habits.
- Program records: call notes, messages, check-in forms, payments.
5.3 Why we use it. To decide whether coaching suits you and is safe for you; to plan and deliver your coaching and track your progress; to arrange your program, including scheduling, payments, receipts and sending CGM sensors; and to meet legal and accounting requirements. We only use your information for other purposes, such as marketing or testimonials, with your separate consent.
5.4 Who we share it with. Only the Coach reads your health information. We never sell your data. We use trusted service providers to run the program, such as our website and form host, email, video calls and messaging. They handle your data only to provide their service to us. We'll only share your information with your doctor or anyone else with your permission, or where the law requires it.
5.5 Data stored outside Singapore. Some of these providers store data on servers outside Singapore. Where that happens, we choose providers with recognised data protection standards so your data is protected to a standard comparable to the PDPA.
5.6 How long we keep it. Answers submitted through our online forms are moved into your private client file and deleted from the form system within 14 days. We keep your client file while you're a client and for up to 2 years after your program ends, unless you ask us to delete it sooner. Payment records are kept as long as the law requires.
5.7 Keeping it safe. We use password-protected accounts with two-step sign-in where available, limit access to the Coach, and avoid sending health details by ordinary email or chat where possible. If a data breach is likely to cause you significant harm, we will tell you and the Personal Data Protection Commission as the PDPA requires.
5.8 Your rights. Email may@my1life.co to see the personal data we hold about you and how it has been used, to correct anything that's wrong, or to withdraw your consent or ask us to delete your data. We'll respond within 30 days. If you withdraw consent for us to use your health information, we won't be able to continue coaching you, and refunds follow section 2.
5.9 Clients outside Singapore. If you live in the EU or UK, you also have rights under the GDPR, including data portability and the right to complain to your local data protection authority. Your consent to the use of your health information is given explicitly for that purpose.
5.10 Age. Our coaching programs are for adults aged 18 and over.
6. Photos, testimonials and marketing
Saying no to any of these doesn't affect your coaching, and you can change your mind at any time by emailing may@my1life.co.
- Anonymous results (on unless you opt out): MY1Life may share your results without your name, photo or anything else that identifies you (for example, "a client in her 50s lowered her HbA1c from 6.1% to 5.6%").
- Named testimonial, photos and video (on unless you opt out): MY1Life may ask to share your words with your first name, or photos or video of you from sessions or events. You'll always see the exact wording or image, and give your OK, before anything is published.
- Marketing emails and messages (off unless you opt in): MY1Life will only send you emails about talks, tips and offers, or WhatsApp and SMS marketing messages, if you tick the box to opt in. You can unsubscribe at any time.
If you withdraw consent for photos or testimonials, MY1Life will stop new uses and remove them from its own website and social media within 14 days. Printed materials already distributed can't be recalled.
